SAIL strives to support transition – age youth with improving workplace skills to improve independent living and nursing home residents with transitioning to community housing options of their choice.

Employment Services Advocates

The Employment Services team works closely with Michigan Rehabilitation Services vocational counselors to assist youth with disabilities learn effective skills to be successful in the workplace. These services may include but are not limited to:

  • Survey for Success assessment
  • Work readiness assessment
  • Benefits planning assessment
  • Independent Living assessment
  • Career Scope
  • Work readiness training
  • Work/personal adjustment peer mentoring
  • Post-secondary peer mentoring and/or independent living assessment
  • Job search techniques, resume review and preparation

  • Summer work readiness program

Community Transition Services Program

Through collaboration with the State of Michigan and funded through the Center for Medicaid Services (CMS), SAIL strives to identify and transition individuals living in nursing home facilities who would be better served to live independently in the community. SAIL provides information and supports for people who want to move from nursing homes to the housing option of their choice.

  • Identify individuals who do not meet the level of care and would like to transition out of the nursing facility. This program would support the individual and their family in providing support and information to return to a home of their choice.
  • Assist individuals in developing a Community Transition Assessment (CTA) that helps to outline their person-centered plan for transition
  • Assess and support the individual in identifying a barrier to independent living
  • Evaluate housing options, assess potential barriers and find ways to overcome them through referrals to local housing services providers
  • Prepare for transition by identifying needs for support, transportation, and community activities
  • Follow individuals once they transitioned into the community to ensure success
